Worked example
A product extracts the normal sample well but cannot return the original file beside a corrected field. If reviewer traceability is mandatory, that gap remains a disqualifier even when the weighted feature score is high.
The useful evidence is the comparison between expected and actual behavior. Record corrections and rerun the same case after a change so the team can see whether the fix worked.
